Vision Source is unable to guide generating reports from your Practice Management System. If you are unable to locate or generate the required reports, contact your Practice Management System vendor for assistance.
It is the practice's responsibility to obtain the required data needed for quarterly reporting. Failure to submit the required supporting documentation will result in ineligibility for Member Dividends 50 for the applicable reporting quarter.
